Health & Vaccination screening for campus visits
Before coming to campus, all staff, faculty members and students must provide (1) proof of vaccination and (2) complete a daily self-assessment on Ucheck. Only those who receive a ‘green’ status are permitted to be on-campus. At any time, you may be asked to show your green status, therefore, please have it readily accessible. A paper based assessment form is also available, however, filling out the form does not replace the requirement to complete the Ucheck Vaccine Declaration on the Ucheck web portal.
Please note that it is expected that all members of the community have their first COVID-19 vaccine dose by Sept. 13 and their final dose by Oct. 15 in order to qualify as fully vaccinated by Oct. 29. If you receive a red status on Ucheck, you must not come to campus- please isolate at home and following the appropriate steps. Students who receive a red status should immediately connect with their campus health centre (St. George: 416-978-8030) to determine next steps, including whether the Occupational Health Nurse must be contacted. When accessing any indoor space on campus, you must wear a mask as per the University’s Policy. For more information regarding masks, please see the Joint Provistial and Human Resources Guideline on Face Masks.
Currently, research spaces (for approved lab members), faculty offices and rooms used for the purposes of classroom instructions are accessible. For accessibility to other spaces or the use of a space for an event, please consult email@example.com. With the exception of indoor instructional settings (i.e. classrooms, teaching laboratories, etc) where physical distancing is not required, everyone must maintain a minimum of 2m physical distancing, in addition to following other COVID-19 protocols (masking, Ucheck, etc).
